Observed History and Physical

Students are required one fully observed H&P (or 4 different portions of H&P’s) during the rotation. Evals can be done in clinics as well as inpatient. Residents or faculty can complete the task. At least one completed H&P form should be turned into the clerkship coordinator by the end of the Clerkship rotation.

ECG Clinical Skill

Students will complete a clinical skill rubric for ECG interpretation during their rotation. Student must be directly observed by a faculty member or a resident.

*This is a pass/fail assignment. Student must achieve a rating of "able to perform independently" in order to pass.

Midpoint Evaluations

These evaluations should serve as a guide for continued improvement over the course of the Clerkship. They are intended to provide specific feedback including praise for strengths and recommendations for improvement.

1. Print the evaluations below and have an Attending and Resident complete it
2. Go over the evaluation with the evaluator for direct feedback and discussion
3. Return the signed evaluation to Dr. Maddox at the Midpoint Meeting
